Revised Class V(W) Malfunction and Defect Reporting

1. The purpose of this MARADMIN is to provide an immediate update to the procedures contained in reference (a) for reporting malfunctions and defects associated with Class V(W) materiel.  This updated process does not change the reporting process For any mishap involving ammunition and explosives meeting the criteria of reference (b).
2. I MEF submitted an Iterative Transformation Initiative (ITI)  titled "Malfunction and Defect Reporting Modernization" to the Ammunition Logistics Focus Team (ALFT) within PM Ammo. The ITI requested the current requirement to report Class V(W) Malfunctions and Defects via the Automated Message Handling System (AMHS) process be modernized to simplify reporting.
3. A "Flash Malfunction/Defect report", with pre-populated fields in excel format, was developed.  Guidance was given to submit the report via email to the organizational mailboxes of the offices who evaluate malfunctions and defects within the Marine Corps.  The guidance was clear to ensure the respective units chain of command is also copied.
4. A pilot program was conducted to test the effectiveness and efficiency of this process.  Within the first few months, the quality, quantity and timeliness of the reports increased exponentially.  An added benefit of emailing the reports is the ability to attach pictures of the malfunctions or defects, when possible.  The evaluators are reporting the pictures greatly assist in the expeditious assessment process.  During the pilot program it was also recommended that the spreadsheet-based "Flash Malfunction and Defect Report", be replaced with .pdf fillable NAVMC forms to ensure configuration management of the forms is maintained.
5. NAVMC Form 11893 (Ammunition Malfunction Report) and NAVMC Form 11894 (Ammunition Defect Report) were formally published by HQMC (ARDB).
6. The NAVMC forms can be found on the Marine Ammunition Knowledge Enterprise (MAKE).  To access the MAKE site, any user with a Common Access Card (CAC) can create an account using the following url:  https:(slash)(slash)make.mceits.usmc.mil. The NAVMC forms are in the "library" sub directory under the heading "Malfunction/Defect Reporting".
7. To enhance the safety of our Marines, this MARADMIN serves as notification that, effective immediately, all Marine Corps units shall use email reporting, utilizing the NAVMC forms for both Initial and supplemental Class V(W) Malfunctions and Defects.  Emails should be addressed to accounts identified in the NAVMC instruction block.  All other policy and procedures, besides the reporting process, within reference (a) remain germane.
8. This MARADMIN applies to the total force.
