Parachute Incident Immediate Notification Reporting Procedures
This MARADMIN establishes supplemental email-based immediate notification procedures for parachute mishaps, hazards, and near-miss incidents to ensure timely dissemination to senior Marine Corps leaders and parachuting stakeholders. Units conducting parachute operations must submit DD Form 1748 reports within 24 hours (Class A-C mishaps) or 48 hours (Class D-E mishaps and hazards) to CMC Safety Division, which will disseminate information via a Microsoft Teams distribution list. This process supplements, but does not replace, existing Risk Management Information (RMI) system reporting requirements.

1. This message establishes supplemental reporting procedures for parachute mishaps, hazards, and/or incidents to enhance timely dissemination to senior leaders and parachuting stakeholders, in addition to required RMI reporting per Refs A and B. This message applies to all Marine Corps units conducting Low-Level Static Line, Military Free Fall, Tandem Offset Resupply Delivery System ā Equipment and Personnel, and Cargo Operations. 2. Background 2.a. The Marine Corps uses the Risk Management Information (RMI) system as the official platform for parachute mishap reporting. However, the reliance on users to log in and actively monitor RMI has led to delays in fleet-wide awareness when parachuting incidents occur, affecting leaders' ability to make risk informed decisions. To address this, an email-based notification process has been established to rapidly disseminate information. 3. Definitions 3.a. Mishap. A mishap is an unplanned or unexpected event or series of events that results in damage to DoD property; work-related illness to DoD personnel; injury to on or off-duty DoD military personnel; injury to on-duty DoD civilian personnel; or damage to non-DoD property or injury or illness to non-DoD personnel, caused by DoD activities. See Ref A for definitions of Class A - E mishaps. 3.b. Hazard. Any real or potential condition that can cause injury, illness or death to personnel or damage to or loss of equipment or property or mission degradation. 3.c. Ref A further defines a near-miss under the hazard definition as: An act or event (i.e., near miss) that may have resulted in a mishap where the fatality, injury, illness, property damage or loss of an asset was avoided merely by chance, the actions of an individual or individuals, a small measure of distance or a few moments in time. 4. Action 4.a. Immediate Reporting Procedures 4.a.1. The parachute mishap email notification process does not replace RMI, the Dive/Jump Reporting System (DJRS), or the DD Form 1748. 4.a.1.a. All Class A through E mishaps involving Cargo and Parachuting, as well as HAZREPs (incidents or malfunctions with no injury or damage), must be reported through RMI. 4.a.1.b. To ensure rapid dissemination and operational awareness, all Class A through E mishaps, hazards, and near-miss events shall also be reported via email immediate notification per this policy. This ensures timely dissemination to senior leaders and key stakeholders across the FMF and supporting establishment. 4.a.1.c. Units shall ensure key personnel at all echelons have appropriate RMI access, permissions, and training. 4.a.2. Once the DD1748 is complete, the unit shall submit the DD1748 via DJRS to CMC Safety Division. Upon receipt of the DD1748, CMC Safety Division will verify no Personally Identifiable Information (PII) is included and is directed to disseminate the information to the email distribution list. 4.a.3. The DD1748 shall be submitted by the unit experiencing the mishap within the following timelines: 24 hours for Class A through C mishaps and 48 hours for Class D, Class E, and hazard (near-miss) events. 4.a.4. The Immediate Notification Email shall follow a standardized format: Who (unit involved), What (brief description of the event), Where (location of the event), When (date/time of occurrence), parachute configuration, and a brief narrative summary of the event. 4.b. Microsoft Teams Page 4.b.1. A Microsoft Teams page titled "Parachute Incident Notifications" will be established by the HQMC PP&O Reconnaissance Occupational Field Manager to establish a distribution list. 4.b.2. Each organization listed in paragraph 4.c. requires at least two personnel to be assigned as page owners for their unit. Units will provide those names to the POCs listed above and access will be granted. These individuals will manage access and membership for their respective units and ensure appropriate visibility of reports. 4.b.3. Page owners are responsible for maintaining an up-to-date roster of users within their unit. This includes ensuring new joins are added and those who depart the unit are removed in a timely manner. 4.c. Distribution The following organizations are required to designate personnel for the distribution list and access to the Teams page: 4.c.1. CMC Safety Division 4.c.2. Reconnaissance Battalions (1st Recon Bn, 2d Recon Bn, 3d Recon Bn, and 4th Recon Bn) 4.c.8. Deputy Commandant, Combat Development and Integration (DC CD&I) 4.c.9.
